It's a limited edition palette from Urban Decay (2011)
16 shades (from left to right, top to bottom):
-Blue bus, Gunmetal, Cobra, Baked, Bender, Gravity, Lost, Hijackn, Midnight cowgirl, Sin, Midnight rodeo, Crystal, Burst, Missionary, Skimp (the one with the hole in it) and Zephyr.
They also give a waterproof eyeliner and an eyeshadow primer potion (not in the pictures).
My opinion: I absolutly love it! I got it for christmas 2011 and I use it everyday!
